New Patient Registration

The Delamere Medical Practice is accepting new patients who live within the M32 postcode area. To register with the practice you will need to complete the following Forms that you will see at the bottom of the page. Alternatively please call into the practice to complete a new patient registration pack.

New temporary and permanent patients to the practice will need to provide photographic identification and proof of address as part of the registration process. A current passport or photo driving licence will be accepted for identification and recent utility bill (within the last 3 months) for proof of address. Please bring in details of any medication that you are currently taking as well. For children, a birth certificate will be required and, where possible, a passport, if you are filling in the forms online please email us over a copy of your identification.

Upon completing your registration process at the Delamere Medical Practice, you will be assigned a named accountable GP, however you have the flexibility to see any of the GPs for routine appointments. In addition we ask you fill in our E-consult to book appointments.

We also have access to a small number of evening and weekend appointments through the Trafford hub. These hub appointments are not at the Delamere medical practice but at other practices within Trafford, please ask at reception if you require one of these appointments.
